Kirstin (00:11):
We're back with more hot takes from Dr. Koehler. All right, well let's get to our favorite stuff. Surgery.

Dr. Koehler (00:20):
Yes, let's talk about that.

Kirstin (00:22):
Yes. Okay. So one trend on social media right now is neck and chin lipo.

Dr. Koehler (00:32):
That's a trend?

Kirstin (00:34):
Yeah.

Dr. Koehler (00:35):
Like it's been trending for 40 years probably. It's not new.

Kirstin (00:38):
Oh well.

Dr. Koehler (00:39):
That's just something new?

Kirstin (00:41):
No, it's not new. It's just people are talking about it and people are making other people, influencers are getting it, making other people want to get it.

Dr. Koehler (00:49):
Yeah. I mean, we touched on that already, but neck liposuction, if a person has fat that sits between the skin and the muscle in the neck, you absolutely can do neck liposuction. I'm not saying that nobody's a candidate. There are, but sometimes it's a combination problem and, okay, not a great analogy, but I'm going to use it anyhow. But we have women who come in sometimes after pregnancy and they've got bulging abdomen and they want a tummy tuck. And if the problem is the muscle separation and skin laxity, that the muscle separation is causing the abdomen to bulge forward because the muscles aren't in the right spot, then yes, a tummy tuck will fix their problem. But sometimes they have a bulging abdomen and they might have some muscle separation, but they have another problem. And that other problem is internal visceral fat. And that visceral fat is what's making the abdomen bulge.

(01:47):
And so even though we would tighten the muscles, they're still going to have a bulging abdomen. So similar thing with the neck, if the problem is just the fat, superficially, yes, I can improve the contour of the neck. But if the problem is deeper to that and it's the deeper fat that's pushing things forward and down, you have to do a different procedure. And now in this case, there is a procedure we can do and we can tighten the muscles and get to the deep fat. So having an experienced doctor who can maybe identify when you're a better candidate for either procedure is good, but what you don't want to have is like, okay, you get liposuction in the neck and then you can barely see a change, and the reason is it really was the deeper problem.

Kirstin (02:28):
Okay.

Dr. Koehler (02:31):
So it's patient selection. And I would say that I'm more inclined to do things surgically than, and liposuction is still surgery, but it's less invasive.

Kirstin (02:43):
What about the next one?

Dr. Koehler (02:46):
The next one?

Kirstin (02:47):
It is buccal fat pad removal.

Dr. Koehler (02:51):
What about it?

Kirstin (02:52):
Do we like it? Yeah.

Dr. Koehler (02:55):
In the right patient. Always in the right patient. So if a person has a very full face and this cheek region's really full, and we're trying to maybe get a little bit more of a definition, not so rounded, then taking out that fat pad can be effective. I think you got to be careful. It's not like if somebody's already got a thinner face, you don't want to do that. You're going to make 'em look skeletor. It's not, or maybe not, even if they say, maybe they're just a younger patient and they just have an average, it's not really rounded or full. And we're also talking about not an obesity issue, it's just your face is fuller. If it's a weight issue that's not, you can take the buccal fat pad out, but what if they lose weight? I mean, there's a lot of other questions there, but the thing about buccal fat is, you can only reduce so much. And you have to be careful on people who are, maybe their faces are thinner because you will make them look, maybe not at that age, but down the road, they're going to look hollowed out and it'll make 'em look older, not younger.

Kirstin (04:07):
Good to know. The next trend that we've seen on social media lately is cat eye or a fox eye lift. And this is where it's kind of a brow lift.

Dr. Koehler (04:17):
We don't treat cats. We are moving on to humans. I don't make cats. We don't create cats. We don't treat cats. We're done.

Kirstin (04:25):
No veterinarians at Eastern Shore Cosmetic Surgery.

Dr. Koehler (04:28):
No. Get off my lawn. I don't care to, cat, I mean, if somebody asks me to give 'em cat eyes, I will send them to the cat doctor. I am not. No, we don't do that.

Kirstin (04:37):
Get off my lawn.

Dr. Koehler (04:38):
Yeah.

Kirstin (04:39):
